Is it rare? -
I don't actively collect support chips, so I don't know how rare it is. When I bought it I thought that these are not rare. I looked at cpu-zone, theintelcollection, and a couple of other sites and couldn't find any without "C" prefix.

I would think it was extremely rare!
I just looked up George Phillips Guide to Vintage Intel Chips and he does not show any without the "C" prefix so I would say it would have a value of US$300 plus.

That's a pretty nice find.
What I am just wondering is the strange combination of part number and program code number.
I've never seen one with a "-", btw, 0207 is pretty early (I guess they started with 0001 and just counted up). My lowest number is 0226 on a gold pinned P4001 with no date code.

| I assume many of you know that the Intel 4001 is a mask-programmable ROM for the 4004 microprocessor (MCS-4 family). The four digits after the dash indicate which custom-made metal mask was used to program the ROM. I do not know whether they were truly sequential, but I can confirm that they did start at -0001.
