after many years . . . . . . I found it!
It have still the serial label!
I have always believed that the writings were adhesive, instead they is varnish. The white cover, I believed painted the original black cover, instead it is plastic white more thinner. _________________ Fatti non foste a viver come bruti, ma per seguir virtute e canoscenza.

Great find! A real rarity, if only 150 were produced! Certainly rarer than most of the "rare" chips we see on eBay  _________________ My graphics card database can be found at http://www.gpuzoo.com.
