Hello,
I don't know in which devices it was used, but here is a board on which this IC lives.
It has the following product numbers and terms on it, all printed in golden letters :-) :
A-1212-4
08755-60001
Shaping, gain, offset

well i have see the board ... pulled from the same one 
Must be some old analog Scientific instrument from h/p or something |

I tried to find more information on this board, but this is pretty difficult.
Googling just the first part of the product number leads to:
HP 8755C Swept Amplitude Analyzer
But I have no idea if this information is correct. There are no pictures which show the internals. :-( |
