These are some 4Mbit EEPROM from DensePAC, made with 4 Atmel AT28C010-12JI industrial spec 1MBit EEPROMs on a ceramic Substrate.
Also on package is a milspec 54C138 multiplexor.
The entire MCM is industrial temp rated.
