Mouse and 2 pads.
Mouse has superficial scratches on case, No wear on bottom.
One mouse pad has scratch, The other has worn spots.
Looks like Xerox Alto-1 but the optical sensor is a cerdip and not plastic Has penciled-in part #/ lot? Looks like a Hawley design.

NOT Serial - This mouse is quadrature output

Was unused from 80's CAE workstation manufacturer Metheus Corp. for Lambda IC design workstation.
