Perhaps the largest is the Fujitsu M-1800, or the Fujitsu Numerical Wind Tunnel MCM? They appear to be 10" (255mm) square -- until you open the heat sink frame. Counting their flange they are 12" (305mm) square.

This may be one of the biggest, I saw it on a german computer forum, Fujitsu M1800 MCM, appears to be about 230mmx230mm square

From my research I agree -- the Fujitsu M-1800 [and it's subsequent version, the Numerical Wind Tunnel MCM] are indeed the largest MCMs ever made.
I am having difficulty finding photos that are small enough to post here... have a few.

I have a great comparison photo of both the NEC SX-3 (top) and the Fujitsu M-1800
but don't know how to make the photo small enough to meet the 350KB limit
will look into how to do that

found a way to make the photo smaller
perhaps too small
but it does show the NEC SX-3 on top
the Fujitsu M-1800 on bottom

the NEC is 10"x10" in its frame, but less than 9"x9" by itself
whereas the Fujitsu MCM is as large as its frame (due to flanges) and is 11"x11"
