what exactly is a 'qualification sample'? some kind of test card to see if perform well?

As it's a stage between ES and production, I would say ES is like alpha testing, QS is more like beta testing. So I would think it's samples sent to a wider audience than ES.

yup typically sent out to OEMs to test and often to media outlets as well

"Qualification Sample" for a card has very broad meaning.
My understanding is: imagine that you plan to manufacture e.g. new line of budget PCs,
and you have a motherboard made specifically for you.
During development stage you still need to test a card that you plan to use in it,
no matter how long it is in production.
And manufacturer (this time ATi itself) gives you this sample.

Most likely a regular GPU.
And - unisial feature, it have active cooler!
Usually they were without cooling at all or have passive one.
Afaik active coolers were used only starting with Rage 128 Pro/Ultra.

In nearly all cases, QS are just free samples sent to customers for validation. They have exact same specification as production part (it would be useless to validate a design with anything different than the "real" thing). Lot of them are just production parts with a special sticker. Some are marked "Not for sale", some "Qualification Sample".

at least memory chips on it are ES Smile

i tried it and it's working.. Win XP detects it as Xpert 128 .. only when i tested it i noticed it's on PCI not AGP Smile))

The date of production is also known - September 4, 1999. Such cards are not intended for sale and are used mainly during testing by the manufacturer. The core frequency in it is reduced from the nominal 103 MHz to 90 MHz, and on the board you can find some additional elements that are missing from the consumer versions."
