I'm reasonable sure (99+%) it was late 1974, months after the 8080 was released, despite the myriad of sites which claim it was 1972... I have a table of microprocessors published in July of 1974 that lists the 4040 as "rumored" and gives its tentative part number as 4014.
I don't have a '73 or '74 Intel data book, but it's definitely not in the September '72 edition. Does anyone know for certain when it came out?

I have 1972 introduction date for the 4040 on the site. I think I've got it from the "Great Microprocessors of the Past and Present" and never verified it.
